Fo-Ti (also known as Polygonum multiflorum or He Shou Wu) has been used in Traditional Chinese Medicine (TCM) for centuries, primarily for its reputed anti-aging properties and to promote longevity, vitality, and hair growth. Its use to support or treat osteoporosis is grounded in traditional beliefs rather than robust scientific validation. In TCM, Fo-Ti is thought to "nourish the kidney and liver," organs believed to play a crucial role in bone health and strength. As osteoporosis is considered to be related to "kidney deficiency" in this system, Fo-Ti has sometimes been included in herbal formulas intended to strengthen the bones.

However, modern scientific evidence supporting Fo-Ti’s efficacy for osteoporosis is sparse and inconclusive. There are very few animal studies suggesting possible bone-protective effects, but these are preliminary, limited in scope, and do not provide sufficient evidence for clinical recommendations. Human studies are lacking. Therefore, while Fo-Ti’s use for osteoporosis is justified by tradition, it is not supported by strong scientific research. The evidence rating is low, reflecting the absence of high-quality clinical trials or consistent mechanistic data.
