Rehmannia glutinosa is a traditional Chinese medicinal herb that has been used in formulas to support bone health and treat conditions associated with bone weakness, such as osteoporosis. The use of Rehmannia is rooted in traditional Chinese medicine (TCM), where it is considered to nourish the “kidney,” an organ system believed to govern bone strength. However, beyond traditional use, there is some emerging scientific evidence supporting its potential benefit for osteoporosis.
Preclinical studies (mainly in vitro and animal models) have indicated that extracts of Rehmannia, particularly its component catalpol, may promote osteoblast (bone-forming cell) activity and inhibit osteoclast (bone-resorbing cell) activity. These effects are thought to occur via modulation of pathways such as the RANKL/OPG system, which is central to bone remodeling. Some studies have also reported that Rehmannia can help mitigate bone loss in ovariectomized rats, an established animal model for postmenopausal osteoporosis.
However, robust human clinical data are lacking. Most evidence comes from laboratory and animal studies, and the translation to clinical efficacy and safety in humans remains unproven. Therefore, while there is a scientific rationale and preclinical support for the use of Rehmannia in osteoporosis, the overall quality and quantity of evidence is limited, justifying a modest evidence rating.
