Somatropin is a recombinant form of human growth hormone (hGH) and has been investigated and used in the management of osteoporosis, especially in cases related to adult growth hormone deficiency (GHD). The scientific rationale is based on hGH's effects on bone metabolism: it stimulates osteoblast activity, increases bone formation, and indirectly promotes bone growth through the production of insulin-like growth factor 1 (IGF-1). Several randomized controlled trials and meta-analyses have shown that somatropin therapy in adults with GHD can increase bone mineral density (BMD) and may reduce fracture risk over time. However, the magnitude of benefit is moderate, and the evidence is strongest in those with documented GHD rather than in the general osteoporotic population. Current guidelines do not recommend somatropin as a first-line treatment for osteoporosis unrelated to GHD, as bisphosphonates, denosumab, and other agents have a stronger evidence base for fracture reduction in postmenopausal and senile osteoporosis. In summary, there is moderate scientific support for somatropin's use in osteoporosis in the context of GHD, but its use is limited and not broadly endorsed for osteoporosis in the absence of hormone deficiency.
