Agnusides are iridoid glycosides found primarily in the fruits of Vitex agnus-castus, commonly known as chaste tree or chasteberry. The use of Vitex agnus-castus to support ovarian and reproductive health, particularly in women, is well-established in traditional European herbal medicine and has been the subject of modern scientific inquiry. Clinical studies have investigated extracts standardized to agnusides for their effects on premenstrual syndrome (PMS), luteal phase defects, and menstrual cycle regulation. The proposed mechanism involves modulation of pituitary hormones, especially prolactin, which can influence ovarian function and menstrual regularity.

A 2014 review in Planta Medica and several randomized controlled trials have shown that Vitex extracts can reduce symptoms of PMS and mastalgia, and may support corpus luteum function by decreasing prolactin levels, with some studies attributing these effects to agnusides. However, while the evidence is promising and there is biological plausibility, most studies use whole extracts rather than isolated agnusides, and variability in formulations and methodologies limits definitive conclusions. Therefore, while the use of agnusides (via Vitex extracts) to support the ovarian system is supported by both traditional and scientific evidence, the direct evidence for isolated agnusides is less robust, justifying a moderate evidence rating.
