Bromelain is an enzyme mixture extracted from pineapples, primarily known for its proteolytic (protein-digesting) properties. There is moderate scientific evidence supporting its use for general pain, especially pain associated with inflammation, such as that seen in osteoarthritis, sports injuries, and post-surgical recovery. Clinical studies and meta-analyses have indicated that bromelain may reduce pain and swelling by modulating inflammatory mediators and reducing the production of prostaglandins involved in pain signaling.

Several randomized controlled trials have demonstrated that oral bromelain supplementation can reduce pain and improve function in people with knee osteoarthritis, at doses ranging from 500 to 2000 mg per day. Some studies have compared bromelain favorably with traditional non-steroidal anti-inflammatory drugs (NSAIDs) in terms of pain reduction, though results are mixed and often limited by small sample sizes and study design limitations. Bromelain is thought to exert its analgesic effects mainly through anti-inflammatory mechanisms rather than direct pain suppression.

Historically, bromelain has also been used in folk medicine for wound healing and to reduce swelling after injuries or surgeries, which often translates to pain relief. However, more robust, large-scale studies are needed to fully establish its efficacy and optimal dosing. Overall, bromelain is considered a promising adjunct remedy for pain management, particularly where inflammation is a contributing factor.
