Silymarin, an extract from the milk thistle plant (Silybum marianum), is primarily known for its hepatoprotective (liver-protecting) properties. However, emerging preclinical and limited clinical research suggests that silymarin may offer benefit in the context of pancreatitis, mainly due to its antioxidant, anti-inflammatory, and antifibrotic effects. Experimental studies in animal models of acute and chronic pancreatitis have shown that silymarin can reduce oxidative stress, decrease inflammatory cytokine production (such as TNF-α and IL-6), and attenuate tissue damage in pancreatic tissue. These effects are attributed to the compound’s ability to scavenge free radicals and modulate inflammatory signaling pathways.

Some small-scale clinical studies have examined silymarin as an adjunct therapy in patients with chronic pancreatitis, reporting improvements in symptoms and reductions in markers of inflammation and oxidative stress. However, these studies are limited by small sample sizes, short durations, and methodological weaknesses. There is currently insufficient high-quality clinical evidence to recommend silymarin as a standard treatment for pancreatitis, but its favorable safety profile and promising preclinical results have led to its experimental use in some regions.
