Alpha-Glycerylphosphorylcholine (Alpha-GPC) is a cholinergic compound used primarily for its cognitive-enhancing properties. In the context of Parkinson's Disease (PD), some scientific rationale exists for its use, largely due to its role as a precursor to acetylcholine, a neurotransmitter often deficient in neurodegenerative conditions. A small number of clinical studies have investigated Alpha-GPC in relation to cognitive impairment in PD patients. For example, a few open-label and small randomized controlled trials conducted in the late 20th century reported mild improvements in cognitive and neuropsychological parameters in PD patients with dementia when Alpha-GPC was administered as an adjunctive therapy, sometimes alongside standard dopaminergic medications or cholinesterase inhibitors. However, these studies were limited in size, duration, and methodological rigor, and most were published in Italian or other non-English journals. Systematic reviews and meta-analyses have not established Alpha-GPC as a standard or widely recommended treatment for PD, and major clinical guidelines do not mention its use for PD motor or non-motor symptoms. While there is a plausible mechanism and limited clinical support for cognitive symptoms, the overall quality and quantity of evidence are low, resulting in a modest evidence rating.
