Vitamin E (tocofersolan, a water-soluble form of vitamin E) is used as a supportive treatment for peripheral neuropathy, particularly in cases related to vitamin E deficiency. The scientific justification for this use is based mainly on evidence that vitamin E deficiency can cause neurological symptoms, including peripheral neuropathy. Most data come from case reports, studies in individuals with malabsorption syndromes, genetic defects affecting vitamin E transport (such as abetalipoproteinemia), or chronic cholestatic liver diseases, where vitamin E supplementation (including tocofersolan) can halt or sometimes reverse neuropathic symptoms.

However, for peripheral neuropathy not caused by vitamin E deficiency (such as diabetic neuropathy or chemotherapy-induced neuropathy), evidence supporting the use of vitamin E—including tocofersolan—is limited and inconsistent. Systematic reviews and randomized controlled trials have not convincingly shown benefit in these populations. Major guidelines do not recommend vitamin E for most forms of peripheral neuropathy unless there is established deficiency.

Tocofersolan is specifically used in patients with fat malabsorption, where standard fat-soluble vitamin E formulations are poorly absorbed. Thus, its use is scientifically justified in deficiency states, but not as a general therapy for all peripheral neuropathies. The strength of evidence supporting its routine use outside deficiency states is low.
