Lumbrokinase is a group of proteolytic enzymes derived from earthworms (Lumbricus rubellus) and is primarily known for its fibrinolytic (clot-dissolving) activity. Its use in supporting or treating phlebitis—an inflammatory condition of the veins, often associated with thrombosis—has some basis in scientific literature, although the volume and quality of evidence are limited. The rationale behind its use stems from its ability to enhance fibrinolysis, potentially reducing clot formation and aiding in the dissolution of existing clots, which are key factors in the management of thrombotic phlebitis.

Several in vitro and animal studies have demonstrated that lumbrokinase can break down fibrin and reduce blood viscosity, suggesting a possible benefit in thrombotic disorders. A small number of clinical studies, mostly published in Chinese journals, have reported positive outcomes in conditions involving thrombosis, including some types of phlebitis. However, these studies often suffer from methodological limitations such as small sample sizes, lack of proper controls, and potential publication bias.

Despite these limitations, lumbrokinase is used in China and some other countries as an adjunct therapy for various thrombotic conditions. Western clinical guidelines do not currently recommend it, and larger, high-quality randomized controlled trials are needed to confirm its efficacy and safety for phlebitis. Thus, while there is some scientific rationale and preliminary evidence for its use in phlebitis, the overall strength of evidence is low to moderate.
