27-Deoxyactein is a triterpene glycoside found primarily in Actaea racemosa (black cohosh), a plant traditionally used by Native American and European herbalists to support women’s reproductive health, including the alleviation of premenstrual syndrome (PMS) symptoms. The use of black cohosh for PMS is based mainly on its long history in traditional medicine, where it was employed for a variety of menstrual complaints, although it is more famously associated with menopausal symptom relief. The actual constituent, 27-deoxyactein, is thought to be one of several active compounds in the plant, but studies directly examining its effects are limited.
Scientific support for black cohosh’s efficacy in treating PMS specifically is sparse and not robust. Most clinical studies and reviews focus on menopausal symptoms rather than PMS. While some animal and in vitro studies suggest that black cohosh extracts (containing 27-deoxyactein) may have mild estrogenic or serotonergic activity, these effects have not been clearly linked to improvements in PMS symptoms in well-designed human trials. Therefore, the rating reflects the reliance on traditional use, with only limited indirect evidence from broader studies on black cohosh, and a lack of specific scientific validation for 27-deoxyactein’s role in PMS.
