European peony (Paeonia officinalis) has a long history of traditional use, particularly in European and Chinese medicine, for addressing various menstrual and gynecological complaints, including symptoms related to premenstrual syndrome (PMS). PMS Type D refers to the "depressive" subtype, characterized by mood swings, irritability, and depressive symptoms. In traditional herbal medicine, peony root has been used as a calming agent and to regulate menstrual cycles. Classical European herbal texts, as well as some records from Traditional Chinese Medicine (where a related species, Paeonia lactiflora, is more commonly used), describe peony as beneficial for "female complaints," including mood disturbances associated with menstruation.
However, direct modern scientific evidence supporting the use of European peony specifically for PMS Type D is limited. Some phytochemical studies indicate that peony contains compounds with mild sedative and anti-inflammatory effects, which might theoretically help with mood symptoms. Nonetheless, most existing research on PMS and peony focuses on Paeonia lactiflora in the context of formulas like "Shaoyao-Gancao Tang," and even those studies are preliminary. Therefore, the use of European peony for PMS Type D is primarily justified by historical and traditional practice, with limited scientific validation to date. Its evidence rating is modest due to the lack of robust clinical trials specifically addressing the efficacy of Paeonia officinalis for PMS-related depressive symptoms.
