Evening primrose oil (EPO) has been traditionally used to support symptoms of premenstrual syndrome (PMS), particularly PMS Type H, which is characterized by "hyperhydration" or water retention, bloating, and breast tenderness. The rationale for its use stems largely from its high content of gamma-linolenic acid (GLA), an omega-6 fatty acid believed to modulate inflammatory pathways and prostaglandin production. Historical use of EPO for women’s health issues, including menstrual discomfort and breast pain, is well established in herbal medicine traditions, especially in Europe and North America.
However, scientific evidence supporting EPO’s efficacy for PMS is limited and inconsistent. Several small studies and reviews have investigated EPO for various PMS symptoms, with some reporting mild benefit for mastalgia (breast pain) and bloating, but others showing no significant difference from placebo. Most clinical trials have methodological limitations, such as small sample sizes, lack of blinding, or inconsistent diagnostic criteria. Systematic reviews and meta-analyses generally conclude that there is insufficient high-quality evidence to recommend EPO as an effective treatment for PMS or PMS Type H.
In summary, while EPO is commonly used and recommended in traditional and alternative medicine circles for PMS Type H symptoms, robust scientific validation is lacking. Its continued use is primarily justified by tradition rather than strong clinical evidence.
