Vitex agnus-castus, also known as chaste tree berry, is commonly used for the management of premenstrual syndrome (PMS), including PMS Type H, which is characterized by symptoms related to water retention such as bloating, breast tenderness, and weight gain. Scientific evidence suggests that Vitex agnus-castus can be effective for these symptoms. The proposed mechanism involves modulation of prolactin levels via dopaminergic effects, which may alleviate breast tenderness and fluid retention. Several randomized, placebo-controlled trials have demonstrated reductions in overall PMS symptoms and mastalgia (breast pain) in women treated with Vitex compared to placebo.
Notably, a systematic review and meta-analysis published in 2017 (Cochrane Database Syst Rev) concluded that Vitex agnus-castus extract is more effective than placebo for reducing PMS symptoms, with the strongest effects seen in symptoms related to breast pain and fluid retention. However, most studies have limitations such as small sample sizes, short durations, and variable formulations of Vitex extracts. Despite these limitations, the consistency of positive findings across studies provides moderate evidence for its efficacy in treating PMS symptoms, especially those associated with PMS Type H.
