Chastetree (Vitex agnus-castus) is widely used to manage symptoms associated with premenstrual syndrome (PMS), including PMS Type P (characterized by predominant symptoms of pain, such as breast tenderness and headaches). The scientific rationale for its use is based on its dopaminergic effects, which help regulate prolactin secretion. Elevated prolactin is often implicated in PMS symptoms, notably breast pain and swelling. Several randomized, placebo-controlled clinical trials have evaluated the efficacy of Chastetree extract in reducing PMS symptoms. A meta-analysis published in 2017 in the journal Phytomedicine concluded that Vitex agnus-castus is significantly more effective than placebo in alleviating overall PMS symptoms, particularly mastalgia (breast pain) and psychological symptoms. The proposed mechanism involves modulation of the hypothalamic-pituitary axis, leading to normalization of luteal phase defects and reduction in excessive prolactin release. The German Commission E has approved the use of Chastetree for irregularities of the menstrual cycle, premenstrual complaints, and mastodynia. While more research is needed to clarify the most effective dose and extract standardization, the cumulative evidence provides a strong scientific basis for recommending Chastetree as a supportive treatment for PMS Type P.
