Vitex agnus-castus (chaste tree berry) is primarily used to support symptoms of premenstrual syndrome (PMS), including what is often termed PMS Type P (characterized by predominantly psychological symptoms such as irritability, mood swings, anxiety, and tension). The scientific evidence for Vitex in the treatment of PMS is moderate. Several randomized controlled trials and meta-analyses support its efficacy in reducing the overall severity of PMS symptoms, with a notable effect on mood-related symptoms.
The proposed mechanism of action involves Vitex’s dopaminergic effects, which in turn reduce prolactin release from the pituitary gland. Elevated prolactin levels have been associated with PMS symptoms, especially mood disturbances. By modulating prolactin, Vitex may help restore hormonal balance during the luteal phase of the menstrual cycle. A systematic review published in Phytomedicine (2017) and another in the Cochrane Database (2012) concluded that Vitex is more effective than placebo and possibly as effective as some standard treatments (e.g., pyridoxine) in managing PMS symptoms, though more large-scale, high-quality studies are needed.
Historically, Vitex has been used in European herbal medicine for centuries to address menstrual disorders, but its use for the psychological symptoms of PMS has gained scientific support only in recent decades. Adverse events are usually mild and rare. Overall, the evidence base is moderate but promising regarding Vitex’s effect on PMS Type P symptoms, particularly for mood disturbances.
