Chaste tree (Vitex agnus-castus) is widely used to support or treat PMS, particularly PMS Type S (characterized by symptoms such as swelling, breast tenderness, and bloating). Scientific studies have demonstrated that chaste tree can be effective in alleviating these symptoms, largely owing to its dopaminergic effects, which decrease excessive prolactin secretion from the pituitary gland. Elevated prolactin is associated with many PMS Type S symptoms, especially mastalgia (breast pain and swelling). Several randomized, double-blind, placebo-controlled clinical trials have shown that chaste tree extract significantly reduces PMS symptoms compared to placebo, with particular benefit for breast tenderness and fluid retention.

The mechanism is thought to involve the herb’s ability to bind to dopamine receptors in the pituitary, thereby inhibiting prolactin release. A 2017 systematic review and meta-analysis concluded that chaste tree is superior to placebo in reducing overall PMS symptoms, with the strongest effects seen in PMS Type S complaints. The safety profile is generally favorable, with mild and transient side effects. While additional large-scale studies would be beneficial, the current clinical evidence, combined with long-standing traditional use in Europe, supports the use of chaste tree for PMS Type S. Thus, chaste tree is both a traditional and scientifically validated remedy for this condition, with a strong body of evidence supporting its efficacy.
