Immunoglobulin G (IgG) is primarily used in the treatment of pneumonia in specific clinical contexts, particularly in cases related to immunodeficiency. Scientific evidence supports the use of intravenous immunoglobulin (IVIG), which is largely composed of IgG, for patients with primary immunodeficiencies (such as Common Variable Immunodeficiency or X-linked Agammaglobulinemia) who are highly susceptible to recurrent bacterial infections, including pneumonia. In these patients, IVIG supplementation provides the missing antibodies necessary for effective immune responses, thereby reducing the frequency and severity of infections, including pneumonia. Several clinical trials and guidelines (e.g., from the American Academy of Allergy, Asthma & Immunology) validate this use. However, for the treatment of pneumonia in immunocompetent patients, there is no robust evidence supporting the routine use of IgG or IVIG. Its application in pneumonia outside of immunodeficiency is generally limited and not standard practice. In certain severe or refractory cases (e.g., in some forms of severe viral pneumonia or in immunocompromised patients), IVIG may be considered as adjunctive therapy, but evidence is less consistent and recommendations are cautious. Thus, the scientific support is strong in the context of immunodeficiency-related pneumonia, but limited otherwise.
