Folinic acid, also known as leucovorin, is a form of folate (vitamin B9) that is sometimes used as a supplement during pregnancy. The scientific rationale for using folinic acid, rather than folic acid, is based on its ability to bypass certain metabolic steps required for folic acid activation. This can be particularly important for individuals with genetic polymorphisms, such as MTHFR mutations, which impair the conversion of folic acid to its active form. Adequate folate is critical during pregnancy to support fetal neural tube development and prevent neural tube defects (NTDs). While folic acid is the standard recommendation, folinic acid may be considered in special populations where folic acid metabolism is compromised. However, direct comparisons between folinic acid and folic acid for pregnancy outcomes are limited, and folic acid remains the primary supplement recommended.
