Alpha phytosterol, more commonly referred to as beta-sitosterol as part of the phytosterol family, has been studied for its potential benefits in supporting prostate health, particularly in men with benign prostatic hyperplasia (BPH). Multiple randomized, placebo-controlled clinical trials and meta-analyses have found that phytosterols—especially beta-sitosterol—can modestly improve urinary symptoms and flow measures in men with BPH. The most cited evidence comes from a 1999 meta-analysis published in the journal British Journal of Urology that concluded phytosterols improved urinary symptom scores and flow measures compared to placebo, although the effects were not dramatic and the studies were of short duration. The proposed mechanisms include inhibition of 5-alpha-reductase and anti-inflammatory effects. However, it is important to note that most research focuses on mixed phytosterols or beta-sitosterol specifically, and the evidence for alpha phytosterol alone is not robust. The overall strength of evidence is moderate (rated 3/5), as findings are consistent but not overwhelmingly strong, and longer-term, larger studies are lacking. Despite these limitations, the use of phytosterols for prostate health enjoys some scientific support and is not based solely on tradition or anecdote.
