Dendramine is an antihistamine, better known by the generic name diphenhydramine (commonly marketed as Benadryl). Its use in the treatment of rashes and hives (urticaria) is strongly supported by scientific evidence. Diphenhydramine is a first-generation H1 histamine receptor antagonist, and it works by competitively inhibiting the action of histamine at H1 receptors, thereby reducing the allergic response in tissues, which manifests as itching, swelling, and redness—key features of rashes and hives.

A number of clinical guidelines (such as those from the American Academy of Dermatology and the World Allergy Organization) endorse the use of oral antihistamines, including diphenhydramine, for the symptomatic relief of urticaria and allergic dermatoses. Several randomized controlled trials and reviews have demonstrated that diphenhydramine is effective in reducing pruritus (itching), erythema (redness), and wheal formation in patients with acute allergic skin reactions. However, due to its sedative effects, second-generation antihistamines are often preferred for long-term or daytime use, but diphenhydramine remains a validated option, especially for acute or nighttime symptom relief.

Topical formulations of diphenhydramine are also available for localized skin reactions, though their use is less well-supported due to the risk of sensitization.
