Sanguisorba, particularly Sanguisorba officinalis (great burnet), has a long-standing history of use in traditional Chinese medicine (TCM) and some Western herbal traditions for treating various skin conditions, including rashes and hives (urticaria). In TCM, Sanguisorba (known as "Di Yu") is classified as an herb that cools the blood and stops bleeding, and it is often prescribed for ailments characterized by heat and inflammation in the skin, such as eczema, rashes, and allergic eruptions. The rationale is based on the herb’s reputed ability to clear “heat-toxins,” reduce inflammation, and promote tissue healing. Some historical European sources also mention its use for wound healing and as an astringent for minor skin irritations.

However, scientific validation for these specific uses is limited. There are a few in vitro and animal studies suggesting that extracts of Sanguisorba possess anti-inflammatory, antioxidant, and hemostatic properties, which could theoretically benefit skin inflammation. Still, robust clinical trials in humans for rashes or hives are lacking. The Western herbal literature tends to emphasize its astringent and wound-healing properties rather than direct treatment of allergic skin reactions. Therefore, while its use is justified by tradition and historical precedent, it is not currently well supported by high-quality scientific evidence for the treatment of rashes and hives.
