11-Keto-beta-boswellic acid (KBA) is one of the active triterpenoid compounds found in Boswellia serrata resin, commonly known as frankincense. Scientific interest in KBA and other boswellic acids has grown due to their purported anti-inflammatory properties. A number of preclinical studies (in vitro and in animal models) have demonstrated that KBA inhibits 5-lipoxygenase, an enzyme involved in leukotriene synthesis, which plays a key role in the inflammatory cascade associated with rheumatoid arthritis (RA). Some studies also suggest that KBA can modulate cytokine production and reduce oxidative stress.

However, when it comes to clinical evidence, the data is limited and of moderate quality. A few small randomized controlled trials and open-label studies involving Boswellia serrata extracts (which contain KBA among other boswellic acids) have reported reductions in joint swelling, pain, and improvement in physical function in patients with RA. The specific contribution of 11-keto-beta-boswellic acid, as opposed to the whole extract or other boswellic acids, is less clear, as most clinical formulations use the whole resin or standardized mixtures.

In summary, while there is a scientific rationale and some preliminary clinical data supporting the use of 11-keto-beta-boswellic acid for RA, high-quality, large-scale clinical trials are lacking. Therefore, the evidence can be rated as modest (2/5), and its use is better characterized as supported by emerging science rather than strong clinical proof.
