Mangiferin is a naturally occurring polyphenolic compound found primarily in mango leaves and bark. Its use for rheumatoid arthritis (RA) is primarily supported by preclinical scientific evidence, rather than longstanding traditional use or robust clinical trials. Several in vitro and animal studies have demonstrated that mangiferin exhibits anti-inflammatory and antioxidant properties, which are relevant to the pathogenesis of RA. For example, research has shown that mangiferin can inhibit the activation of nuclear factor-kappa B (NF-κB) and reduce the expression of pro-inflammatory cytokines such as TNF-α, IL-1β, and IL-6—molecules implicated in RA progression. Additionally, mangiferin has been shown to attenuate joint swelling and histopathological damage in animal models of arthritis. However, it is important to note that, as of the current knowledge cutoff in 2024, there are no published high-quality randomized controlled trials in humans specifically evaluating mangiferin as a treatment for RA. The available evidence is limited to laboratory and animal studies, which, while promising, do not provide sufficient basis for clinical recommendations. Therefore, while there is a scientific rationale and preliminary evidence for its use, the overall level of evidence supporting mangiferin’s effectiveness in RA remains low, justifying a rating of 2/5.
