Terpinen-4-ol is a major component of tea tree oil (Melaleuca alternifolia) and has been studied for its acaricidal (mite-killing) properties, including against Sarcoptes scabiei, the causative agent of scabies. Laboratory studies have demonstrated that terpinen-4-ol exhibits significant in vitro activity against scabies mites. For instance, research published in peer-reviewed journals (e.g., Walton et al., 2004; Carson et al., 2013) has shown that concentrations of terpinen-4-ol can kill scabies mites and eggs, sometimes more rapidly than standard treatments such as permethrin. Additionally, there have been small clinical reports and case series suggesting that tea tree oil-based formulations, and by extension terpinen-4-ol, may be effective in managing scabies infestations and associated symptoms such as itching and secondary infection.

However, large-scale randomized controlled trials are lacking, and most clinical evidence comes from small studies or anecdotal reports, not from terpinen-4-ol as a single isolated agent but rather as part of tea tree oil preparations. Nonetheless, the scientific basis for its use is supported by laboratory and limited clinical data, justifying a moderate rating for evidence. Conventional therapies remain the first-line treatments, but terpinen-4-ol is considered a promising adjunct or alternative, especially in cases of resistance or intolerance to standard scabicides.
