Paeonia moutan (commonly known as moutan peony or tree peony) has a long history of use in traditional Chinese medicine (TCM), where it is valued for its purported effects on "blood stasis" and inflammation. In TCM, herbs that "move blood" and reduce stasis are occasionally applied topically or taken internally to support the healing of scars or to soften scar tissue. Paeonia moutan root bark (commonly known as Mu Dan Pi) is one such herb, traditionally included in formulations for skin health and the management of scars. However, while these practices are documented in historical materia medica texts, specific studies or controlled clinical trials demonstrating the efficacy of Paeonia moutan for scars or scar tissue are lacking.

Modern scientific literature primarily focuses on the anti-inflammatory, antioxidant, and immunomodulatory properties of Paeonia moutan, rather than direct effects on scar formation or remodeling. To date, there is only very limited preclinical research (mostly in vitro or in animal models) examining its effects on fibroblast activity or collagen deposition, which are relevant to scarring, and these findings are not robust or specific enough to constitute scientific validation. In summary, the use of Paeonia moutan for scars is based in tradition rather than science, and the supporting evidence is weak.
