Elderberry (Sambucus nigra), especially in the form of syrups or extracts, has a long history of traditional use in European and North American herbal medicine for treating respiratory ailments, including symptoms resembling those of sinus infections (sinusitis). Folk medicine has used elderberry to address colds, flu, and upper respiratory tract infections because of its purported antiviral and anti-inflammatory effects. The berries and flowers are thought to reduce mucous membrane inflammation and support immune function, which could theoretically help with the congestion and discomfort associated with sinus infections. However, while several small studies have examined elderberry's potential for reducing the duration or severity of influenza and common colds, there is very limited direct scientific evidence supporting its efficacy specifically for sinus infections. Most available research focuses on elderberry's general effects on upper respiratory tract infections rather than sinusitis per se. Consequently, its use for sinus infections is primarily rooted in tradition and extrapolated from its use for other respiratory illnesses. No large, high-quality clinical trials have directly evaluated elderberry for acute or chronic sinusitis. Therefore, while traditional use is well-documented, scientific validation for elderberry's role in sinus infection treatment is weak and indirect.
