Japanese Elder (Sambucus sieboldiana) is traditionally used in East Asian herbal medicine, though its use for sinus infections is not as well documented as that of its European relative, Sambucus nigra (European elderberry). In Japanese and Chinese traditional medicine, extracts from elder plants have long been employed for respiratory ailments, including coughs and colds, which sometimes overlap with sinus issues. However, direct references to its use specifically for sinus infection (sinusitis) are limited. The rationale in traditional usage is generally based on elder’s reputed anti-inflammatory and diaphoretic properties, believed to help "expel wind" and clear congestion.

Modern scientific validation for Japanese elder’s efficacy against sinus infections is lacking.
