Oldenlandia, particularly Oldenlandia diffusa, has a long history of use in traditional Chinese medicine (TCM). In TCM, it is commonly referred to as "Bai Hua She She Cao" and is traditionally used for its anti-inflammatory, detoxifying, and heat-clearing properties. Although most of its traditional applications focus on infections, fever, and even cancer support, it has also been used as a component in herbal blends intended to relieve respiratory ailments, including sinus congestion and sinus infections (rhinosinusitis). The rationale is largely based on the herb's perceived ability to "clear heat and toxins," which, in TCM theory, are often implicated in sinus infections. However, there is limited direct scientific validation for Oldenlandia's efficacy specifically in treating sinus infections. Some in vitro and animal studies suggest anti-inflammatory and antimicrobial effects, which could theoretically be beneficial for sinus issues, but these studies do not directly address clinical outcomes in human sinusitis. There are no high-quality clinical trials evaluating its use for sinus infections specifically. Therefore, while traditional use is established and there is some preliminary pharmacological rationale, robust scientific validation is lacking. The rating reflects the strong traditional basis but only modest scientific support for use in sinusitis.
