Pelargonium, specifically the species Pelargonium sidoides, has been studied for its potential in treating respiratory tract infections, including acute sinusitis. Several randomized controlled trials and meta-analyses have investigated its efficacy. The extracts used in these studies are typically standardized (not unspecified), most often the proprietary extract EPs 7630. Evidence suggests that Pelargonium sidoides can have a modest beneficial effect in reducing the duration and severity of symptoms in acute, uncomplicated sinusitis, likely due to its antimicrobial, immune-modulating, and anti-inflammatory properties.

However, the overall quality of the evidence is moderate at best, with some studies showing only small effects. The European Medicines Agency (EMA) recognizes traditional use for mild upper respiratory conditions, and some German and European guidelines mention it as an option. Nonetheless, major clinical guidelines in the US and UK do not recommend Pelargonium for sinus infection due to insufficient high-quality evidence. Importantly, "Pelargonium (unspecified)" encompasses many species, and only P. sidoides has been researched; evidence does not extend to other species or non-standardized preparations. Therefore, while there is some scientific support, it is limited in strength and scope.
