European elderberry (Sambucus nigra) has a long history of use in traditional herbal medicine, particularly for supporting respiratory health, including sinus-related complaints. More recently, scientific interest has focused on its bioactive constituents, such as flavonoids (including quercetin and anthocyanins) and triterpenes, which have demonstrated antioxidant, anti-inflammatory, and antiviral properties in laboratory and animal models. A few small human clinical trials and observational studies have shown that elderberry extracts may reduce the duration and severity of symptoms in upper respiratory tract infections, such as the common cold and influenza. These effects are thought to be partly due to the inhibition of viral replication and modulation of cytokine production, which could indirectly benefit the sinuses by reducing inflammation and congestion.

However, while there is moderate scientific evidence supporting elderberry's role in alleviating general upper respiratory symptoms, direct research specifically targeting sinus health (e.g., sinusitis) is limited.
