Guaco (Mikania glomerata or Mikania laevigata) is a plant native to South America, particularly Brazil, and has a long history of traditional use in herbal medicine for respiratory ailments, including those affecting the sinuses. Traditionally, guaco leaves are used in teas, tinctures, or syrups to help relieve symptoms of colds, coughs, asthma, and sinus congestion. The primary constituents thought to be responsible for these effects are coumarin and other related compounds, which are believed to have bronchodilatory, anti-inflammatory, and expectorant properties.

While there is some pharmacological research indicating that guaco may have mild bronchodilatory and anti-inflammatory effects, especially in animal models and in vitro studies, there is limited high-quality clinical evidence supporting its efficacy specifically for sinus health or sinusitis in humans. Most of the available evidence is anecdotal or comes from traditional medicine practices in Brazil and neighboring countries. Thus, its use for supporting the sinuses is primarily justified by tradition rather than robust scientific validation, and the overall evidence level remains moderate to low.
