Honey loquat syrup, known in Chinese as "pei pa koa," has a long tradition of use in East Asian medicine for soothing the throat, relieving coughs, and supporting respiratory health, including the sinuses. The primary ingredients, honey and loquat (Eriobotrya japonica), are traditionally believed to have moistening and cooling properties. In traditional Chinese medicine (TCM), loquat leaf is thought to clear heat and transform phlegm, which is conceptually linked to supporting healthy sinuses and respiratory passages. Honey is also valued for its soothing and demulcent effects on mucous membranes.

However, while there is a solid historical basis for using honey loquat syrup in this context, modern scientific studies directly supporting its efficacy for sinus health are limited. Some studies have investigated the anti-inflammatory and antimicrobial activities of loquat leaf extracts and honey, suggesting possible benefits for respiratory tract irritation and infection, but these are not specific to sinus health and are mostly preclinical. Most clinical evidence is anecdotal or based on small studies in cough relief rather than direct sinus support. As such, the use of honey loquat syrup for sinus support is primarily justified by tradition, with only modest scientific backing for its broader respiratory soothing effects.
