Several double-blind, placebo-controlled trials have examined standardized butterbur extracts (such as Ze 339) in patients with allergic rhinitis, which often involves sinus symptoms. These studies have shown that butterbur extract can reduce symptoms such as nasal congestion, sneezing, and itchy eyes, with efficacy comparable to some antihistamines but with fewer sedative effects. The proposed mechanism involves inhibition of leukotriene synthesis, which plays a role in inflammation and allergic responses. However, it is important to note that raw butterbur contains pyrrolizidine alkaloids (PAs), which can be toxic to the liver, so only PA-free extracts should be used.
