Umckalin is a natural coumarin compound found in the roots of Pelargonium sidoides, a plant widely used in traditional South African medicine. Modern scientific interest in Umckalin and Pelargonium root extracts (marketed as Umcka or EPs 7630) centers on their use for acute respiratory tract infections, including sinusitis. Clinical studies and meta-analyses have shown that standardized extracts of Pelargonium sidoides can modestly reduce the duration and severity of symptoms in acute sinusitis and bronchitis. The proposed mechanisms include antimicrobial, antiviral, and immunomodulatory effects, with Umckalin considered one of several active constituents contributing to these effects. However, most human studies have tested the whole root extract, not isolated Umckalin. Regulatory agencies in Europe recognize Pelargonium extract as a traditional herbal remedy for respiratory tract infections, and some clinical guidelines note its possible benefit for acute, uncomplicated sinusitis. While there is scientific support for the use of Pelargonium extracts in sinus support, the direct evidence specific to Umckalin alone is limited. Thus, the overall evidence for Umckalin in supporting the sinus system is moderate (rated 3 out of 5), primarily derived from studies of the whole plant extract rather than the isolated compound.
