Umckaloabo, also known as Pelargonium sidoides, is a traditional herbal remedy from South Africa that has been increasingly studied for its effects on respiratory tract infections, including those involving the sinuses. Scientific studies, including randomized controlled trials and meta-analyses, suggest that extracts of Pelargonium sidoides can be effective in reducing the duration and severity of symptoms in acute respiratory tract infections, such as acute bronchitis and the common cold, both of which often involve the sinus passages.

The mechanism of action is believed to involve immunomodulatory effects, mild antimicrobial activity, and enhancement of the body's mucociliary clearance. Several clinical trials have demonstrated statistically significant improvements in symptom scores (such as nasal congestion, headache, and sinus pressure) among adults and children treated with standardized Umckaloabo extracts compared to placebo. For example, a 2008 meta-analysis published in Phytomedicine concluded that Pelargonium sidoides extract is effective for acute bronchitis and may have benefits for sinusitis, although direct research specific to chronic or isolated sinusitis is limited.

While these results are promising, the body of evidence is not as robust as that for some conventional pharmaceutical treatments, and research specific to chronic sinusitis is lacking. Nonetheless, Umckaloabo is used in Europe and elsewhere in standardized extract form as an over-the-counter remedy for acute upper respiratory tract infections, reflecting a growing scientific basis for its use in supporting the sinuses and related systems.
