Alkanna tinctoria, commonly known as alkanet, has a long history of traditional use for treating skin conditions, particularly infections, wounds, and ulcers. In various folk medicinal systems, especially in the Mediterranean, Middle Eastern, and South Asian regions, its root is applied topically as a poultice or ointment to promote healing and prevent infection. The main active compounds in Alkanna include naphthoquinones such as alkannin and shikonin, which are responsible for its characteristic red pigment.
There is some in vitro scientific evidence that alkannin and shikonin possess antibacterial, anti-inflammatory, and wound-healing properties, which could support the traditional uses; however, robust clinical trials in humans are lacking. Most of the available data come from cell culture or animal studies, and these have shown that alkannin derivatives can inhibit the growth of certain bacteria and promote tissue repair. Nevertheless, these findings have not yet translated into widespread acceptance in evidence-based medicine, and regulatory agencies do not currently approve Alkanna-based preparations for treating skin infections in clinical practice. Thus, while traditional use is substantial, scientific validation remains limited, meriting a moderate evidence rating.
