Benzyl benzoate is primarily used as a topical treatment for scabies and, to a lesser extent, for pediculosis (lice infestations), both of which are parasitic skin infections. The scientific validation for its use comes from multiple clinical studies and longstanding recommendations by health authorities. Benzyl benzoate acts as an acaricide and pediculicide, effectively killing the mites (Sarcoptes scabiei) and lice responsible for these infections. It is included in the World Health Organization’s Model List of Essential Medicines for the treatment of scabies.

Clinical trials and observational studies have shown that benzyl benzoate is effective in eliminating mites when applied as a topical lotion, usually as a 10–25% preparation. It is considered a cost-effective alternative to other scabicides, such as permethrin or ivermectin, especially in resource-limited settings. However, it can cause skin irritation or burning, particularly in children or on sensitive skin. While resistance has been reported in some areas, it remains a widely used and recommended treatment in many regions globally.

Its use is not supported for non-parasitic bacterial or fungal skin infections, and there is no significant evidence suggesting efficacy for such conditions. The primary scientific support is for its use against parasitic skin infections, particularly scabies and lice.
