Lactobacillus acidophilus is a probiotic bacterium commonly used to support gastrointestinal health, including in the context of Small Intestinal Bacterial Overgrowth (SIBO). The scientific rationale for its use is based on the idea that introducing beneficial bacteria may help restore a healthy balance in the gut microbiota, potentially inhibiting the growth of pathogenic or excessive bacteria characteristic of SIBO. Some in vitro and animal studies have suggested that L. acidophilus can produce substances such as lactic acid and bacteriocins that inhibit the growth of harmful bacteria. However, direct clinical evidence in humans specifically supporting the use of L. acidophilus for SIBO is limited and generally of low quality. A few small-scale studies and case reports have examined the use of probiotics, including L. acidophilus, in SIBO patients, often as adjuncts to antibiotics or as part of multi-strain probiotic formulations. Results have been mixed, with some studies showing modest improvements in symptoms or breath test results, while others found no significant effect. No large, high-quality randomized controlled trials specifically evaluating L. acidophilus for SIBO exist as of mid-2024. Thus, while there is a scientific hypothesis and preliminary evidence suggesting a potential benefit, the evidence base is not robust, and guidelines do not currently recommend L. acidophilus as a primary treatment for SIBO.
