Sulforaphane is a naturally occurring isothiocyanate found in cruciferous vegetables, particularly broccoli sprouts. Its primary mechanism of action is the activation of the Nrf2 (nuclear factor erythroid 2–related factor 2) pathway, which regulates the expression of antioxidant and detoxification enzymes. In the context of small intestine health, preclinical studies have shown that sulforaphane may protect intestinal epithelial cells against oxidative stress and inflammation. For example, animal studies and in vitro models have demonstrated that sulforaphane can reduce intestinal permeability (“leaky gut”) and limit pro-inflammatory cytokine production, both of which are implicated in disorders like inflammatory bowel disease (IBD).

However, evidence from human clinical trials is limited. Most studies examining sulforaphane’s effects on gut health have been conducted in animals or cell cultures, and not specifically in the small intestine of healthy humans or those with small intestine-specific disorders. Some small, preliminary human studies have shown that sulforaphane-rich broccoli sprout extracts may have beneficial effects on gut microbial composition and markers of inflammation, but these findings are not yet conclusive.

In summary, the use of sulforaphane to support small intestine health is grounded in promising mechanistic and preclinical evidence, but robust human data are lacking. Therefore, it receives a moderate evidence rating.
