Zinc carnosine (also known as polaprezinc) has been investigated for its potential to support the gastrointestinal tract, including the small intestine. Its use is mainly supported by scientific studies, primarily from Japan and other countries, where it is used as a mucosal protective agent. Zinc carnosine is a chelated compound combining zinc and L-carnosine, which is believed to enhance the stability and delivery of zinc to the gastrointestinal mucosa.

Several clinical and preclinical studies have demonstrated that zinc carnosine can help maintain the integrity of the intestinal mucosa, promote healing of ulcers, and reduce inflammation. Mechanistically, it appears to work by stabilizing the mucosal barrier, reducing oxidative stress, and modulating inflammatory responses. For example, human studies have shown that zinc carnosine can reduce intestinal permeability ("leaky gut") induced by stress or nonsteroidal anti-inflammatory drugs (NSAIDs). Additionally, it may promote the repair of damaged intestinal tissue and support overall gut health.

While much of the evidence comes from studies on the stomach (such as gastric ulcers and H. pylori infection), there is a growing body of research indicating benefits for the small intestine, particularly in the context of mucosal protection and repair. However, large-scale, high-quality randomized controlled trials specifically targeting small intestinal health are still limited, which is reflected in the evidence rating of 3 out of 5.
