Calamus (Acorus calamus), also known as sweet flag, has been traditionally used in various systems of medicine, including Ayurveda and traditional Chinese medicine, for treating sore throat and other throat-related ailments. Historical records and ethnobotanical literature suggest that calamus rhizome was sometimes chewed, or its decoction was used as a gargle to relieve throat irritation, hoarseness, and discomfort. The rationale behind its use stems from its purported anti-inflammatory, antimicrobial, and soothing properties. However, while these traditional practices are well-documented, there is limited scientific research specifically evaluating the efficacy of calamus for sore throat. Most available studies focus on its general antimicrobial and anti-inflammatory activities in vitro or in animal models, rather than direct clinical trials in humans for sore throat. Furthermore, concerns exist regarding the safety of calamus due to the presence of β-asarone, a compound considered potentially carcinogenic. Regulatory agencies in several countries restrict its use in food and medicine.
