Zhejiang fritillary (Fritillaria thunbergii), known in Chinese medicine as "Zhe Bei Mu" (浙贝母), is traditionally used to alleviate sore throat and related symptoms such as cough and phlegm. Its use dates back centuries in Traditional Chinese Medicine (TCM), where it is classified as a "cooling" herb that clears heat and resolves phlegm, often prescribed for "wind-heat" type sore throats, upper respiratory tract infections, and dry coughs. The bioactive compounds in Zhejiang fritillary, such as peimine and peiminine, are believed to have anti-inflammatory, antitussive, and expectorant properties according to some laboratory studies. However, robust clinical trials in humans specifically addressing sore throat are lacking. Most of the available evidence comes from historical texts, TCM formularies, and a few in vitro or animal studies suggesting potential anti-inflammatory effects. As such, while its use for sore throat is deeply rooted in tradition and supported by some preclinical pharmacological data, there is insufficient high-quality scientific evidence (such as randomized controlled trials) to fully validate its efficacy for this indication. Thus, the justification for using Zhejiang fritillary for sore throat is primarily traditional, with a moderate level of supporting evidence.
