Chymopapain is an enzyme extracted from the papaya (Carica papaya) plant, and it has been used in the medical treatment of herniated intervertebral discs, specifically through a procedure known as chemonucleolysis. The use of chymopapain in this context is well-documented in scientific literature, with significant research conducted from the 1960s through the 1990s. Chemonucleolysis involves the injection of chymopapain directly into the herniated disc, where the enzyme enzymatically digests the proteoglycan matrix of the nucleus pulposus, leading to a reduction in disc volume and alleviation of nerve compression symptoms such as pain and sciatica. Multiple clinical trials and systematic reviews have shown that chymopapain can provide significant relief for patients with lumbar disc herniation who have not responded to conservative treatments, with success rates often comparable to surgical discectomy.

However, the use of chymopapain has declined due to concerns about rare but serious adverse reactions, such as anaphylaxis and neurological complications, as well as the development of improved surgical techniques. Despite this, the scientific evidence supporting its effectiveness in appropriate cases is strong, and major medical organizations acknowledged its utility before its withdrawal from the market in many countries.
