Lithospermum erythrorhizon, also known as "Zi Cao" in traditional Chinese medicine, has a long history of use for skin conditions, burns, and wounds. Its application to infections, including those caused by Staphylococcus aureus, is based both on tradition and recent scientific investigations. The main active compound isolated from Lithospermum erythrorhizon is shikonin, which has been studied for its antimicrobial properties. Several in vitro studies have demonstrated that shikonin and related naphthoquinones exhibit inhibitory effects against S. aureus, including some methicillin-resistant strains (MRSA). Mechanisms proposed include disruption of bacterial cell membranes and interference with biofilm formation. Despite these promising laboratory results, clinical evidence in humans remains limited. There are no high-quality randomized controlled trials supporting the efficacy of Lithospermum erythrorhizon or its extracts for treating staph infections in clinical settings. Most available data are preclinical, involving cell cultures and animal models. Therefore, while there is scientific rationale and preliminary evidence for its antimicrobial activity, the level of evidence supporting its use for staph infections is still low, warranting further research to determine its safety and effectiveness in humans.
