Ginkgo flavone glycosides, extracted from the leaves of Ginkgo biloba, have been investigated for their potential neuroprotective and vascular effects, particularly in the context of stroke and cerebrovascular insufficiency. Several clinical studies and meta-analyses suggest that Ginkgo extracts, which contain standardized amounts of flavone glycosides and terpenoids, may provide modest benefits in improving cognitive function and activities of daily living in patients recovering from ischemic stroke. The proposed mechanisms include antioxidant activity, inhibition of platelet-activating factor, vasodilation, and improved microcirculation, which could theoretically aid in reducing brain injury and promoting recovery following a stroke.
However, the quality of evidence remains moderate at best. Most studies have methodological limitations, small sample sizes, or are conducted primarily in China, making it challenging to generalize findings. The Guideline for the Early Management of Patients with Acute Ischemic Stroke (AHA/ASA, 2018) does not recommend Ginkgo biloba as a standard therapy due to insufficient high-quality evidence of efficacy. Systematic reviews (e.g., Cochrane, 2013) conclude that while Ginkgo may improve certain post-stroke outcomes, the risk of bias and inconsistent results limit strong recommendations. Furthermore, safety concerns exist, especially regarding increased bleeding risk due to antiplatelet effects.
In summary, there is some scientific evidence for the use of Ginkgo flavone glycosides in stroke recovery, but the overall strength of evidence is low to moderate, and it is not considered a first-line or standard treatment in clinical guidelines.
