Ginkgo flavone glycosides are the main active components in standardized Ginkgo biloba extracts, such as EGb 761, which are commonly used for a range of vascular and cognitive conditions. Their use in stroke (both prevention and recovery) has some scientific basis, though the evidence is limited and somewhat inconsistent.
Several preclinical studies suggest that Ginkgo extracts may exert neuroprotective effects by reducing oxidative stress, improving microcirculation, and inhibiting platelet-activating factor, which could theoretically benefit patients with ischemic stroke. Some small clinical trials and meta-analyses have reported improvements in cognitive function and activities of daily living in stroke survivors using Ginkgo extracts, especially when combined with conventional therapies. For example, a 2018 multicenter randomized controlled trial published in Stroke & Vascular Neurology found that EGb 761 improved cognitive scores in patients with mild to moderate ischemic stroke.
However, larger and more rigorous studies are needed, as results are not always consistent and some trials have methodological limitations. Furthermore, concerns exist regarding bleeding risk, especially in combination with anticoagulant or antiplatelet medications. Major stroke guidelines do not recommend Ginkgo as a primary or adjunctive therapy for stroke due to insufficient high-quality evidence.
In summary, while there is some scientific investigation into Ginkgo flavone glycosides for stroke, the supporting evidence is weak to moderate at best (rated 2/5), and routine use is not broadly recommended outside of research settings.
