Ginkgoside, a class of active compounds found in Ginkgo biloba extracts, is sometimes used in the context of stroke support or treatment, particularly in China and some other countries. The rationale stems from Ginkgo biloba's purported effects on cerebral blood flow, antioxidant activity, and neuroprotection. Several clinical trials and meta-analyses have evaluated the efficacy of Ginkgo biloba extract (notably EGb 761, which contains standardized amounts of ginkgosides) for cognitive recovery and neurological function after ischemic stroke.
Some studies have shown modest improvements in cognitive performance, neurological deficits, and activities of daily living when Ginkgo biloba extract is used as an adjunct to conventional therapy in stroke patients. For example, a 2018 randomized controlled trial published in Stroke & Vascular Neurology found that EGb 761 improved cognitive outcomes after acute ischemic stroke when combined with standard treatment. However, the quality of evidence is limited by small sample sizes, variable study methodology, and potential publication bias.
Major health organizations do not currently recommend Ginkgo biloba or ginkgosides as a primary or standard therapy for stroke due to insufficient high-quality evidence. While there is scientific investigation into their use, the evidence base is not robust (hence a rating of 2/5). Most guidelines suggest that Ginkgo biloba may be used cautiously and only as an adjunct, not a replacement for established medical treatments.
