Sulbutiamine is a synthetic derivative of thiamine (Vitamin B1) that was originally developed in Japan to treat asthenia (physical and mental fatigue). There is some scientific evidence suggesting that sulbutiamine may help with symptoms related to mental fatigue or "cloudy thinking," sometimes referred to as brain fog. Several small clinical trials and animal studies have demonstrated that sulbutiamine can cross the blood-brain barrier more readily than thiamine, resulting in increased levels of thiamine phosphate esters in the brain. Some studies, including double-blind, placebo-controlled trials, have shown potential benefits in improving subjective measures of fatigue and cognitive performance, particularly in patients with infections, chronic fatigue, or after illness. For example, studies in patients with post-infectious asthenia found that sulbutiamine improved symptoms of fatigue and mental function compared to placebo. However, the number and size of these studies are limited, and the quality of evidence is moderate at best. There is insufficient robust data to firmly establish its efficacy for "cloudy thinking" in otherwise healthy individuals, and most available evidence comes from populations with underlying fatigue or neurological conditions. Overall, while there is some scientific rationale and preliminary data supporting its use for mental fatigue, the evidence is not strong or conclusive.
