Polygonum cuspidatum (Japanese knotweed) has been investigated for its effects on lipid metabolism, including triglyceride levels, primarily because it is a rich source of resveratrol and other polyphenolic compounds. Several animal studies and a few small human trials suggest that knotweed extracts may help lower serum triglycerides. The proposed mechanisms include the modulation of lipid metabolism through activation of AMP-activated protein kinase (AMPK), inhibition of lipid synthesis, and improvement of insulin sensitivity. For example, a 2016 animal study published in Phytotherapy Research found that resveratrol-rich extracts from knotweed lowered triglyceride levels in rats fed a high-fat diet. Some small human studies using resveratrol supplementation (not always directly from knotweed) have also reported modest improvements in triglycerides, but results are inconsistent and sample sizes are limited.

Traditional use of knotweed in Chinese and Japanese medicine has focused more on its anti-inflammatory, antimicrobial, and circulatory benefits, rather than specifically on lipid lowering. Therefore, while there is some scientific rationale and preliminary evidence for knotweed or its constituents in managing high triglycerides, the clinical evidence is limited and not yet robust. Larger, high-quality human trials are needed to confirm efficacy and delineate appropriate dosing and safety. For now, its use for high triglycerides should be considered experimental and adjunctive, not a primary therapy.
