Lovastatin is a member of the statin class of drugs, primarily used to lower cholesterol levels, specifically low-density lipoprotein cholesterol (LDL-C). However, statins, including lovastatin, also have modest effects on lowering triglyceride levels. Clinical studies have consistently shown that lovastatin can reduce triglyceride concentrations, particularly in patients with elevated baseline triglycerides. The typical reduction in triglyceride levels with lovastatin is in the range of 10-25%, depending on the dose and the initial triglyceride concentration.
The mechanism by which lovastatin lowers triglycerides is not completely direct. By inhibiting HMG-CoA reductase, lovastatin reduces cholesterol synthesis in the liver, leading to increased clearance of LDL and, to a lesser extent, very low-density lipoprotein (VLDL), which carries triglycerides. This results in a secondary decrease in plasma triglyceride levels. The effect is generally less pronounced than with fibrates or omega-3 fatty acids, which are considered primary treatments for hypertriglyceridemia, but statins are commonly used in mixed dyslipidemia (where both cholesterol and triglycerides are elevated).
Clinical guidelines recommend statins like lovastatin primarily for LDL reduction, but they are also indicated when triglycerides are mildly to moderately elevated, especially if cardiovascular risk is high. Thus, while lovastatin is not the most potent triglyceride-lowering agent, there is solid scientific evidence to support its use for this purpose, albeit with a moderate effect size.
